The Responder Assistance Initiative (RAI) supports the mental wellness and resilience of public safety and emergency responders across North Carolina by offering behavioral health services, resources, and training grounded in firsthand knowledge of responder culture and best practices. 

We are dedicated to strengthening the resilience and readiness of North Carolia public safety workforce through trusted, culturally informed support and care.

The 988 Suicide & Crisis Lifeline provides free and confidential emotional support to people in suicidal crisis or emotional distress 24 hours a day, 7 days a week, across the United States and its territories. The 988 Lifeline is comprised of a national network of over 200 local crisis centers, combining local care and resources with national standards and best practices.

Outreach

Connecting responder agencies with the available services and wellness resources                   

The Responder Assistance Initiative (RAI) provides outreach to responder agencies across North Carolina to help increase awareness of available services and wellness resources. Through brief, practical program overviews, we help agencies and responders understand what support is available and how to access it.

What OUTREACH can include:

  • Brief overview of RAI services and resources
  • Information on how responders and agencies can access services
  • Participation in agency meetings, shift briefings, or other scheduled gatherings delivered on-site or virtually based on agency preference
  • Conference participation, including staffing information tables and providing brief demonstrations 
Staying Connected

In addition to in-person and virtual outreach, RAI supports responder wellness by sharing helpful information and resources through: 

  • Informational emails
  • Newsletter articles
  • Additional activities designed to promote health and wellness within public safety and emergency response communities

Through these efforts, RAI engages with agencies to better understand their needs and share services and resources that support responder well-being.

Training

Practical training designed for the demands of the field. 

Training plays an important role in supporting the health and resilience of public safety personnel. RAI offers evidence-informed education designed specifically for public safety professionals and the realities of their work. 

What We Offer 

RAI provides practical training on topics that directly impact responder health, resilience, and job readiness. 
Topics include:

  • Stress
  • Trauma
  • Moral Injury
  • Suicide prevention and postvention
  • Peer support

Trainings are tailored to meet the needs of each agency and are designed with responder schedules in mind. 

Designed for Real World Schedules

We understand that responders can’t be out of the field for extended periods of time. Whenever possible, RAI provides flexible training options that work within operational demands. 
Some training courses lead to certification and require consecutive days for successful completion. When this is the case, RAI collaborates with agencies to plan training schedules that balance certification requirements with staffing needs. 

How Training is Delivered
  • Trainings are facilitated by public safety peers, licensed mental health professionals, wellness coordinators, and/or outside subject matter experts
  • Courses are based on best practices and evidence-based curriculum
  • Some trainings require an approved instructor to receive a certificate of completion (Train-the Trainer courses)

Our goal is to ensure agencies have access to relevant, high-quality training that supports the health and readiness of the public safety workforce. Our approach is built on developing partnerships and delivering sustainable support for responder well-being. 

Courses are offered throughout the year and posted in the Training Exercise Response Management System (TERMS).

Behavioral Health Treatment 

Specialized care for the realities you encounter. 

You spend your days showing up for others—running toward chaos, holding the line, and carrying what most people never see. This space is for you.

We offer behavioral health services specifically for first responders, with care that understands the culture, the pressure, and the weight that can come with the job.

What We Offer

Our work is grounded in trauma-informed care, using evidence-based approaches like EMDR and Prolonged Exposure to help process critical incidents, cumulative stress, and the calls that follow you home. We also incorporate Psychological CPR to support stabilization and recovery when things feel overwhelming.

For couples, we draw from Emotionally Focused Therapy (EFT) and the Gottman Method to help strengthen connection, improve communication, and navigate the unique challenges of first responder life. 

Family therapy focuses on supporting the whole system and relationships within the family system, helping families better understand one another, communicate more effectively, and stay connected through the ups and downs. 

Accessible, Flexible, and Free

All services are offered at no cost. Sessions are available in person or virtually, making it easier to access support in a way that fits your life.
You don’t have to carry it all on your own.

Confidentiality 

Your privacy is our priority.  Behavioral Health Services are provided by licensed mental health professionals who are required by federal and state law to protect the confidentiality of your information. 

Any information shared with a RAI licensed provider is considered Protected Health Information (PHI) and will not be released without your written consent. 

There are limited exceptions to confidentiality, including situations involving an imminent risk to yourself or others, or disclosures required by law, such as mandated reporting. In these situations, providers will take the necessary steps to help ensure safety.

Peer Support & Consultation

Real support from people who’ve been there.

Our Approach

Effective peer support starts with strong training, shared experience, and ongoing support. 

RAI builds a well-trained network of peers who use research-based practices while staying true to the heart of peer support: showing up, listening, and walking alongside others. We also recognize that a 2–3-day course alone does not fully prepare someone for this work. 

For that reason, RAI offers ongoing consultation, mentoring, and guidance for newly trained peers. Our goal is to ensure peers feel confident in their role and supported as they provide care aligned with the International Critical Incident Stress Foundation (ICISF) peer support model. 

What Is a Public Safety Peer?

A Public Safety Peer: 

  • Is an active, former, or retired public safety professional
  • Has direct experience with critical incidents and operational stress
  • Has completed accredited peer support training from an approved instructor
Who Serves as a Peer

Peers include law enforcement officers, agents, firefighters, EMTs, dispatchers and spouses. They support others through listening, shared experience, education, guidance, and connection to helpful resources.

How Peer Support Helps Agencies

RAI designs its peer support services to meet agencies where they are whether responding to a single critical incident or addressing ongoing cumulative stress. Agencies have access to experienced public safety peers who bring both professional training and real-world experience. 

RAI Peers provide: 

  • One-on-one peer support
  • Group-based interventions and debriefings
  • Support following critical incidents
  • Consultation with agency leadership
  • Guidance on developing or strengthening peer support programs

Services are confidential and coordinated in partnership with agency leadership as appropriate. 

Confidentiality 

Your privacy is our priority.  Peer support and consultation services are available to address personal issues, and we understand confidentiality is essential.  Peer counseling is considered privileged communication and is protected by law.  

Additionally, RAI providers are not part of the public safety command structure therefore we are not required to share any information with supervisors or leadership.   RAI providers only disclose the necessary information to coordinate referrals or other services.  

There are limited situations in which confidentiality may need to be broken, including concerns about imminent risk of harm to yourself or others, or circumstances requiring disclosure by law. In those situations, appropriate steps will be taken to help ensure your safety. 

Critical Incident Stress Management (CISM) Services 

RAI provides Critical Incident Stress Management (CISM) services to help agencies support responders after difficult calls. 

CISM services may include the following interventions:

  • Informational group sessions
  • Crisis management briefings 
  • Interactive group sessions
  • Critical Incident Stress Debriefings (CISD)
  • Defusings
  • Individual peer support (including peer counseling under General Counsel privilege)
  • Consultation and guidance for agency leadership

RAI coordinates group interventions in collaboration with agency leadership. Planning considers the type of incident, timing, group needs, and responder readiness to ensure services are appropriate and effective. 
CISM services are voluntary and designed to provide education, support, and connection following critical incidents.

Whether navigating cumulative stress, a difficult call, or everyday challenges of the job, RAI peers are there to listen, share perspective, and offer support. Additionally, whether you’re navigating an individual concern or an agency-wide challenge, consultation can provide perspective and practical guidance. Our team is here to help.

Consultation
Expert guidance for the realities of the job. 

Individuals and agencies may occasionally need a trusted behavioral health or peer partner to talk through concerns, explore options, and explore options. Our team provides a neutral, culturally informed resource for responders and agencies navigating complex situations. 
Consultations are confidential and supportive and are not considered psychotherapy. 

Leadership & Agency Consultation

We work with agency leaders to strengthen wellness initiatives and build sustainable support systems for responders.

Examples include:

  • Developing or strengthening internal wellness programs
    • Defining or supporting the wellness coordinator role
    • Suicide prevention strategies
    • Policy and procedure development related to responder wellness
  • Human Factors & Wellness Consultation - Consultation may also focus on challenges that impact responder wellness, performance, or relationships, including:
    • Emotional, behavioral, or performance changes
    • Marital or relationship stress
    • Family medical concerns
    • Legal issues
    • Substance use concerns

These conversations help identify resources, strategies, and supportive next steps.

North Carolina’s public safety professionals serve their communities during some of life’s most difficult moments. The Responder Assistance Initiative (RAI) helps ensure those who serve others have access to the care and resources they need to support their mental well-being. 

Where it Began

In 2019, NCDPS received a two-year, $4.3 million federal grant from the U.S. Department of Justice administered by the Governor’s Crime Commission. This funding helped create the Integrated Behavioral Health Services (IBHS), a program designed to improve access to behavioral health resources for NCDPS employees and their families. 

Building the Program 

A small team worked to establish the program, develop long-term plans, and expanded access to behavioral health services across the department. 

In 2021, state leaders added funding for the program to the state budget. The Governor signed the funding into law, allowing the work to continue to grow. 

Transition to RAI 

That same year state leaders created the Department of Adult Corrections, separating the Division of Adult Corrections from the NCDPS effective January 1, 2023. 

During the transition, the program continued to grow and evolve. Leaders renamed Integrated Behavioral Health Services as the Responder Assistance Initiative (RAI), and the program now operates withing the North Carolina Department of Emergency Management. 

Today

Today, RAI supports first responders across North Carolina, while continuing to serve NCDPS employees and their families. What began as a program for NCDPS staff has grown into a statewide effort that expands access to behavioral health services, strengthens peer support programs, and promote a culture where seeking help is encouraged and supported. 

Looking Ahead

RAI continues to grow through partnerships, outreach, and shared commitment to supporting the mental wellness of North Carolina’s public safety community. As the needs of responders evolve, the program remains dedicated to providing accessible support, practical resources, and a culture where seeking support is encouraged.

When you call the RAI Information & Resource Line you will be asked to leave your name and phone number. A member of the RAI team will return your call to answer your questions or help you to access services.

  • Monday-Friday 7:00 a.m. to 7:00 p.m.
  • Saturday & Sunday 9:00 a.m. to Noon

Calls left after those hours will be returned the next business day.

Emails responses will be sent within 24 hours Monday-Friday 8:00 a.m. to 4:30 p.m.

This confidential, toll-free line provides access to peer support, wellness resources, and clinical support. The line is staffed Monday through Friday from 7:00AM to 7:00PM by RAI providers.  You will be asked to leave a brief message with your name and number. This is not a crisis line.  If you, or the person you are calling about is in imminent danger or has a suicidal or homicidal situation, call 911 or proceed to your nearest emergency room.
